What can I do to prepare for my visit?
At your first visit, you must complete a medical and dental history report. You may find the forms online and fill them out at your leisure prior to your visit. This will streamline your appointment and reduce the time spent in our office.
For the most part, there’s nothing you need to bring. We recommend you wear comfortable clothing, but everything else you need we have available. However, if you don’t complete your paperwork before the appointment, you’ll want to bring a list of your medications and their dosages.
